Something about games that cover the idea of plagues is both intriguing and horrifying to me. On one hand, there’s some pure horror that games can uniquely convey about sickness that are unlike any other form of media. On the other, its intriguing to see the true character of a game that deals with such instances as it offers some insight to how regular people might be affected by a real plague. Perhaps that touches a bit too close to home in the COVID world of today, but its important nonetheless. Pathologic 2 is one of those titles that is able to fully capture these ideas to a terrifying length. Its visceral, real, and an interesting experience that is ‘fun’ only because the gameplay is unique. It’s a huge improvement over its predecessor (which had good themes yet terrible gameplay). Its difficult to fully describe what sets Pathologic 2 apart – its an experience that is somber unlike most others.

Fear Incarnate

Pathologic 2 is a story rich game where you’ve got just 12 days to reunify a town that has been ravaged by disease and their chief medical expert is now dead. My favorite aspect of this game is that there isn’t really a correct way to play it. Sure, you can try to unify the town as I stated previously, attempting to reason and barter with the residents of the town you’ve arrived at. However, that won’t guarantee success. You could just as easily act as a cutthroat rogue to get what you want instead. Either way, your main enemy won’t be the people, or the infected ones: it’s the plague itself. As a result, you’ll need to play tactfully and keep every decision in mind.

It can honestly be hard to recoup a decision you made early on because even that small choice could end up killing you later. I love this sense of freedom because Pathologic 2 doesn’t just throw you into the world without telling you a thing – you get some guidance, but aren’t ‘punished’ for bad decisions. The atmosphere and setting for this game are incredible and remind me frequently of Amnesia while having gameplay elements similar to We Happy Few.

Short on Time

This title is outright grueling at times. I don’t mean that it’s a slog to play through or that it is far too difficult (though it does have its challenges). Pathologic 2 is simply mentally taxing to experience. Its unfair to witness the degradation of this town because it plays out in a realistic manner. You’ll feel helpless most of the time, you’ll struggle to decide between different outcomes, and it’ll just leave you wanting to help everyone.

Bottom Line

There is virtually no filler to be found in this game. Each hour is filled with something meaningful. Even down to the minute you’ll always feel like you’re fighting against time. Again, though this is just a game, it will leave a mental imprint on how you view all games hereafter.


Heavily atmospheric game that has unbeatable writing interspersed. The story can vary and there’s a great potential for replayability – if you can muster the stamina to go through it again.
